Support Coordination
U and V care comprises seasoned Support Coordinators with field experience, well-versed in NDIS systems and services. They collaborate closely with participants and their families to grasp their plans, aiding in provider and service selection. This approach empowers participants and their caregivers, granting them autonomy to pursue their objectives, engage with preferred services, and partake in meaningful activities of their choosing.
We prioritize offering participants a voice and choice in their NDIS plans, ensuring up-to-date communication to facilitate navigation of intricate NDIS and service systems.

- build capacity and capability to understand their plan, navigate the NDIS and make their own decisions.
There are 3 levels of support coordination that can be included in a participant’s plan:
Level 1: Support Connection
Offer guidance to participants in grasping their NDIS plan, linking with providers and community resources, and accessing various supports to optimize plan outcomes. Ultimately, aim to bolster participants’ self-assurance and proficiency in managing their plans autonomously.
Level 2: Coordination of supports
Provide diverse support to enhance participants’ ability to sustain relationships, handle tasks, foster independence, and engage in community activities. This support focuses on empowering participants to take charge of their lives, beyond just managing services, by boosting their confidence and skills.

Assistance with household tasks
such as meal preparation and delivery, house or yard maintenance, cleaning and laundry.
Assistance with daily personal activities
It helps to enable people with more complex needs to live as autonomously as possible, supports are provided by personnel with additional experience/qualifications.
Assistance participants in Daily living and life skills
It includes developmental-focused activities and training to enhance the person’s ability to live as independent an everyday life as possible.
Assistance with Travel and Transport
Provision of Travel arrangements and support for people to access essential services.
Community Nursing care
It includes providing specialist care in the home for those who require a higher level of care and attention.
Participation in the community
Helps in participation activities in the community including supported shopping, medical appointments, sporting and recreational events, etc.
Short Term Accommodation (STA)
Quality respite: Short-Term Accommodation offering a comfortable and relaxed home away from home for individuals with disabilities.
Medium Term Accommodation (MTA)
Support When You Need It: Medium-Term Accomodation assistance for a seamless transition
